Welcome to my practice! You have taken a great first step at working towards a healthier you. I am looking forward to getting to know you at our first session. Please follow these instructions to get ready for our work together.
I conduct a two hour initial session. This is very important in that it allows me to assess what internal mechanisms might be causing your emotional and/or relational problems. It also allows us to determine whether we are a good fit for each other and if my type of therapy will work for you. Often times people experience change and progress just in that first session alone, as the extended time allows us to really dive into what is causing your problems.
If you are going to be using insurance to pay for your sessions, you must contact my billing service, Obrien's Billing prior to your first appointment. The easiest way to provide them with your insurance information is to go to this page and enter the information. They will then let both of us know what your benefit is (e.g., your co-pay, if any; your deductible and how much of it you have already met; and any limits on the number of sessions you are allowed to have).
O’Brien’s also requests a hard-copy of this information, so please bring your insurance card to your first appointment and I will leave their form for you in the waiting room for you to complete.
If you are a self-pay patient, you can simply pay me at each session. My fees are $300 for the initial 2 hour session and $160 for each therapy session after that.
I use an online Electronic Medical Record, Carepaths, which is HIPAA compliant and secure. Once you are my patient you will also be able to log into Carepaths. I ask that you do so prior to your first appointment to complete your initial paperwork. Please follow these instructions:
1. Go to dlp.carepaths.com. At the login page, click the "Sign in" link at the top right of the screen if you do not see the login box. Then, click “patient,” and enter the username and password I provided to you. Once you are logged in you may change your username and password by clicking on the gear icon.
2. Once you have logged in, click on “Clinical Documents”
3. Next, click on “Create New Document” in the upper left portion of your screen.
4. Then, click on each of the five documents and click on, “Create New Document.”
You can then complete the intake forms (Adult Self Report Form and New Patient Information), and read and electronically sign the other forms.
We will schedule your meetings in session when we meet. Depending on both of our availability we may be able to make a standing appointment (the same time every week) but we may not always be able to meet at that time. I also travel frequently, especially in the summer, so if you feel that you need a therapist who can be available every week without any breaks I am probably not the right fit for you.
I video-record my sessions with my patients. Video is a wonderful tool for therapy in that it allows me to be able to review the session after we meet so that I can make sure to provide you the most effective interventions. In other words, if the therapy becomes stuck, I can review the sessions to best determine what to do differently for next time. Therefore, in order to be the most effective therapist that I can be, I require my patients to consent to be recorded with no exceptions.
I record the sessions directly onto an encrypted, password protected hard drive. There are three ways that I may use one of these recordings. The first is for myself to review privately. The second is to show my students a session so that they can learn how to conduct psychotherapy. These are all Ph.D. students in Clinical Psychology at Syracuse University or SUNY Upstate Medical University. They are all therapists themselves, and therefore abide by the ethical code of psychologists, meaning they treat these sessions as they do with their own patients, by maintaining complete confidentiality. They are aware that if they happen to know a patient I am showing them, we stop the video immediately. This teaching is typically done in a small group format with 1-3 students at a time. The third way that I might use the videos are to receive consultation from another licensed therapist. Your video would be kept entirely confidential during this process as the other therapist is also subject to all the rules and regulations regarding confidentiality of medical records.
